Appointments

If you are unwell or need to discuss anything with the Senior Nurse or make an appointment with a Doctor please dial 01332 700455 and select option 2, you will speak directly to a Senior Nurse. We have a number of pre-bookable appointments available, however, once these have gone, you will have to book on the day. We try to book you with the clinician of your choice but this is not always possible. A standard appointment time is 10 minutes.

Routine Appointments

We offer the following services to patients which are bookable through reception on either 01332 700455 (Chellaston surgery) or 01332 861180 (Allenton surgery).

We also hold Chronic disease clinics for patients with Asthma, Diabetes, Hypertension, Heart disease & Chronic respiratory disease, also booked through reception.

Blood taking

These are now carried out by appointment only.

Meadowfields Chellaston 8a.m.- 11:30a.m. Monday,Tuesday, Thursday & Friday

Coleman Health Centre Community Phlebotomy service has a drop-in for blood taking only - open daily 8.30a.m - 12:30p.m.You must have your blood form to use this service.

District Nurses

The District Nursing team provide nursing care to the housebound and patients recently discharged from hospital. Patients requiring a district nurse to visit at home can arrange this through reception.

Counsellor & Physiotherapist

Counselling and physiotherapy sessions are held at the surgery. All referrals for these services are to be made by a clinician.

Health Visitor

The Health visitor team are available to provide health advice for children. They have special responsibility for health and development in the under 5’s and children’s health checks. They may be contacted on 01332 861176. There is a baby clinic held every Tuesday at the Chellaston surgery between 9.00 – 11:00a.m.

Home Visits

Home visit requests should be made by telephoning the triage line on 01332 700455 Option 2 before 10.30am. Please be prepared to give the nurse information relating to your symptoms to help us to assess urgency and timing of visits. Home visits are for patients who cannot physically get to the surgery or are too ill to attend. Lack of transport in itself is not a reason for a home visit.

Information and Access to Records

You may have access to your health records by appointment. There is a charge for this service.

There are times when the practice has to share information held about our patients with other organisations such as hosopitals and social services. This is always done confidentially or by removing identifying details when not essential.

Midwives

The midwives hold regular antenatal clinics and are available to advise on all aspects of maternity care. Appointments can be made through reception. They can also be contacted directly on 01332 861192 between 9am-10am weekdays.

Sicknotes

If you are off work for less than 4 days you do not need a sick note for your employer. For a period off work from 4-7 days, including Saturday and Sunday, you then fill in a self-certification certificate which is available from our reception. If you are off work for more than 7 days then you will need a sick note from the doctor to cover the 8th day of your illness. Please remember that the first 7 days still have to be covered by self-certification. Please call the surgery on 700455 Option 1 to request a sick note. Reception will take the details needed and pass the request to the G.P. Please telephone again before coming to collect your sick note to check that it has been done. Sick notes cannot be issued until the day on which they are due, but can be backdated.

The Department of Work & Pensions states that your employer cannot ask you to provide a sick note for the first 7 days that you are sick. If your employer insists on a sick note to cover absences of 7 days or less the doctor is not able to provide the usual sick note, however we can offer you a private sick note for which there will be a charge.

Test Results

Test results and letters from local hospitals are returned to us each weekday afternoon. The doctor has to see these before the results can be passed on to the patient. If you wish to enquire about a result please telephone the surgery after 3pm on 01332 700455 or 01332 861180 and speak to the receptionist. Most results take at least a week to be available after the test has been taken. Smear results generally take 6-8 weeks to arrive.
